Confidential Communications

1. Who Can Request Your Medical Records?

In order to protect your privacy, only the patient, parent/legal guardian or the patient's legal representative can sign the Authorization for Use and Disclosure form to release medical records. The authorization form must be legible and completed thoroughly and include a copy of your Photo ID in order for us to process your request.

2. How to Request Your Medical Records?

We are happy to provide you with a copy of your medical record within 15 business days as authorized by law. To request a copy of your medical record please follow one of the three options

Medical records will be processed within 15 business days from the date that the completed and signed Authorization for Use and Disclosure form is received. If you need your records immediately or want to pick them up in person, call (888) 499-9303.
