Getting started with MyAltaMed

Manage your health 24/7 – at home or on the go through MyAltaMed. As a ChapCare by AltaMed patient, you will need a MyAltaMed account to access your health information. Through MyAltaMed you can schedule and check in to your appointments, view test results, message your providers, request refill prescriptions, and more. Link your OCHIN account to access both records through one convenient portal.

Getting Set Up Is Easy

Learn more through our video and follow the three easy steps below to set up your MyAltaMed account.

		Start by visiting MyChart.com or you can download the MyChart app. Next, click on the “Access MyChart” button.

		Use the search feature and specify your state.

		Locate the "MyAltaMed" option within the menu to initiate the signup process.

Linking existing OCHIN to MyAltaMed

		Now that you have created your account and logged in, search “Link” in the menu, or press “explore” button on your health feed.

		Search for “OCHIN” in the Discover window.

		Answer verification questions.

Your accounts are now linked. Your OCHIN and MyAltaMed records are now visible in one health feed. OCHIN records will have an identifying icon in the right corner.
