What Is PACE?

PACE—The Program of All-Inclusive Care for the Elderly—is a revolutionary approach to senior care. It was established by Medicare to serve seniors with complex medical needs. Rather than relying on assisted living facilities, PACE creates a comprehensive support system that enables participants to remain safely in their own homes.

PACE provides seniors with coordinated care that addresses their medical, social, and daily living needs. This holistic approach recognizes that true health means maintaining independence, preserving dignity, and fostering community connections. By managing all aspects of care through a single program, PACE eliminates the confusion and fragmentation that often characterizes senior health care.

Comprehensive Health Services

PACE provides a wide array of services designed to address every aspect of senior health and well-being:

  • Coordinated Medical Care Primary care physicians who specialize in geriatrics, specialists, and nurses work together to create a personalized health plan.

  • Social Services Case management and support for navigating benefits and community resources.

  • Adult Day Health Services — Engaging social activities such as games or field trips.
  • Emergency Care Coordination 24/7 access to urgent medical care when needed.

These services work together seamlessly, creating a safety net that allows seniors to age with confidence while remaining active members of their communities.

Who Can Benefit from PACE?

PACE serves independent seniors aged 55 and older with chronic medical conditions who need assistance with requirements of daily living but can still reside safely in their homes. 

Eligibility is limited to those who:

  • Are 55 years or older
  • Currently live within a designated PACE service area
  • Require a nursing home level of care (determined by the state))
  • Can continue to live safely at home through the services provided by PACE

Enrolling in PACE means participants receive all health care services through the program, except in emergencies. This "lock-in" provision ensures coordinated care but requires commitment to working exclusively with PACE providers and in-network specialists.
