AltaMed offers three distinct types of senior services:

Senior BuenaCare

Program of All-Inclusive Care for the Elderly (PACE)

Senior BuenaCare/PACE is a managed-care program that uses an interdisciplinary team approach to provide patients with medical, social, nutritional and rehabilitative services. The Senior BuenaCare program makes it possible for those who are frail and meet nursing home eligibility criteria to live with dignity and pride in their own homes while receiving all the care they need.

PACE offers:

• Primary medical and specialty care
• Prescription medications
• Basic dental care
• All ADHC services
• Hospitalization

Our program serves elderly people with multiple medical problems who:

• Are 55 years old and over
• Need special care and assistance
• Would like to remain in their home
• Are Medi-Cal and/or Medicare eligible or private pay
• Live in the program service area

Adult Day Health Care (ADHC)

Adult Day Health Care (ADHC) centers are designed to help seniors enjoy an independent life while receiving the social and health care services they need in one comfortable enjoyable location. The centers are also critical for caregivers.

Services include:

• Physical, occupational and speech therapy
• Nursing care
• Social worker counseling
• Social and therapeutic activities
• Daily nutrition program (snacks and lunch)
• Transportation to and from the center
• Health education

Senior Care Management

Senior Care Management Programs, including the Multipurpose Senior Services Program (MSSP) and the Integrated Care Management Program (ICMP) are designed to coordinate and provide services to frail and functionally impaired adults.

Services include:

• Home visits
• An individual personal care plan
• Medical and social counseling
• Family support services
• Professional and community referrals
• In-home support services
• Transportation to medical & other appointments
• Continual progress evaluation
• Client advocacy
