PHYSICIAN FAMILY PRACTICE:Provides primary

reports in the development and execution of a

CASE
MANAGER (AFLP Program): Provides
comprehensive case management and outreach services
based on a home-visiting model to eligible pregnant
or parenting teens, some who have Medical and
some who do not. Will assist teens through counseling and referrals to prevent unplanned pregnancies;
assess needs and identify resources available
within the community in order to provide teens
with services and referrals. Will develop Individual
Service Plan (ISP) for each client. As a responsible
team member will contribute to rogram growth,
and will display commitment to the program and
company mission and goals. Requirements: BS (required) in Social Work or other
Social Human Services. Minimum 1 year in case
management work or related experience with adolescents

NURSE,
CASE MANAGER:The
Nurse Case Manager will provide monthly case management
services to an ongoing case load of individuals
living with AIDS. Works as part of an interdisciplinary
team coordination social work and mental health
counseling, psycho-social support services, in-home
support, legal services and skilled nursing to
men and women with AIDS who are homebound. When
applicable, the RN Nurse Case Manager coordinates
activities with the hospital nursing personnel,
with members of the interdisciplinary team and
with the physician in the clinic. The Nurse Case
Manager will report to the Director of HIV Services
or to the Case Management Supervisor, NCM works
in close collaboration with the Divisional Lead
Physician. Oral communication skills are necessary
to give clear, concise instructions to patients
and/or families regarding treatment, to consult
with medical team, to teach and represent recommendations
on programs, and to interact with department staff,
other departments, patients and families with
good public relation skills, and to understand
oral nstructions Requirements: Graduation from an accredited Baccalaureate School
of Nursing or a related filed and two years experience
in administration or management, public health
nursing and/or case management or an RN with three
years experience in home health care and/or case

SUPERVISOR
CASE MANAGEMENT:Under the supervision
of the Site Director the Supervision Senior Services
Counselor/SSSC is responsible for the ongoing
supervision of the care management staff, client
cost monitoring, and the review and authorization
of all client-care service plans. Provides in-service
training for all staff; solves provider/client
issues; and recommends policy changes. The Supervisor
is responsible for quality control, particularly
as related to care management staff. Ability to

OCCUPATIONAL
THERAPIST: Provides an occupational
therapy program for the physical and mental rehabilitation
of participants. Develops and implements Individual
Care Plan for ADHC participants. Coordinates participant
services. Seeking individual with more than 3
years of experience working the frail elderly
population. Evaluates patients, direct patient
treatment, supervision of OT aides and oversight
of treatment provided by OT aides, reassessments,
quarterlies, provides in-services and education. Requirements: Bachelor’s degree
in Occupational Therapy required. Must be licensed
by the California Board of Occupational Therapy.
Occupational Therapy Experience with frail elderly
and disabled populations. Must maintain current

ACTIVITY
COORDINATOR ASSISTANT: Responsible for
developing and implementing an activity program
which meets the needs and interests of each participant.
The activity program shall assist the participant
to resume self-care and normal activities to prevent
further mental or physical deterioration. Requirements: Prefer 2 years of experience in social,
recreational or education program within the past
5 years, 1 year of which was full-time employment
in a patient activities program in a healthcare
setting, mental health setting or setting for
the care of the handicapped. Completion of at
least 36 hours of training in a course designed
specifically for this position and approved by
the Department or on the job training. Must possess
the skill and ability to successfully interact
with seniors; be creative and imaginative in planning,
coordinating, and directing activities. Bilingual

/ or other company sites in a sanitary and attractive
condition. May be expected to make basic repairs. Requirements: Minimum of one

representative of AltaMed programs and services.
Responsible for the outreach and marketing activities;

of a Market-Driven attitude. Capable of learning
and understanding various programs and services
offered within AltaMed. Will work as part of a
team, concentrating on the delivery of high quality
services to all of our customers. Should have
the ability to assess Senior Services and other
patient/referral needs (such as: Family Medicine,
Women’s Health, Pediatrics, HIV/AIDS, etc.). Will

cultivate existing sources; serve as the community

HEALTH
SERVICES ASSOCIATES (Outside Sales & Marketing):
The
Health Services Associate is the community representative
of AltaMed Community Relations Division. The HSA
is responsible for the outreach and marketing
activities; and assists with the development and
implementation of a Market-Driven attitude with
the AltaMed environment. The HSA should fully
understand various programs and services offered
through AltaMed, including Senior Buena Care.
The HSA must work as part of a team, concentrating
on the delivery of high quality services to all
of our customers. The HSA should have the ability
to assess all of AltaMed’s services, including
SBC, and work together with the Community Access
Point Center in order to meet client needs. The
HSA will work to identify new referral sources,
generate enrollments, and increase clinic utilization
as well as cultivate xisting sources. The HSA
will serve as the community liaison for the Community
Relations Division and AltaMed through outreach
activities and coordination with external agencies
and other AltaMed programs. Requirements: BS/BA degree preferred in healthcare
or business administration or a related field

status reports on marketing efforts. Must be familiar
with standard marketing concepts, practices, and
procedures including collateral development, message

effective systems and protocols for standardizing
patient care processes at all clinics. Requirements:This position requires graduation
from an accredited school of nursing, current,
valid licensure by State Board of Nursing, and
5 or more years’ experience in an outpatient
setting. Must have proven ability to implement
policies/procedures across an organization and
maintain a program of compliance. Must also be
able to manage an ongoing training program and
competency assessments. Strong organizational
and written/verbal communication skills required.

ADMINISTRATIVE
ASSISTANT III: The position provides
administrative, secretarial and strategic support
for AltaMed Health Service’s communications,
media relations, and public relations initiatives.
Primary duties include coordinating and executing
press events, writing press releases and other
materials, and developing contacts and relationships
with the media and community partners. Background
in public and community relations. Excellent writing
skills are a must; bilingual English/Spanish is
preferred. An understanding of Medi-Cal, related
public health programs, and the diverse community
that AltaMed serves is a plus. Requirements:
Minimum of 5 years working for a director. Minimum
3 years in media communications, preferably for
a healthcare organization. Experience developing

BUSINESS
INTELLIGENCE MANAGER:Under the direction
of the IT Director, the Business Intelligence
Manager (BIM) is responsible for the design, implementation
and continuing development of the AltaMed data
warehouse (DW) as well as supporting the efforts
of the Business Analysts (BAs) in both a technical
and design/needs analysis role. Additionally,
the BIM will work with the programming team to
design the necessary linkages to existing AltaMed
data sources and apply appropriate business rules
to the data. Requirements: Bachelor’s
degree with major course work in computer sciences,
business administration, or a loselyrelated field.
Five plus years Business Intelligence experience.
Four plus years Supervisory or Management experience.
Four plus years “hands-on” experience in Microsoft-SQL
environment, with expert level use of reporting
and query tools. Three years experience involving
IT support, projects and a variety of computer
applications. Working knowledge of ETL Tools.

INFORMATICS COORDINATOR:Under the direction
of the IT Director, the Clinical Informatics Coordinator
(CIC) is responsible for the design, implementation
and evolution of the AltaMed electronic medical
record (EMR). The primary purpose of this position
is to ensure that the initial design, user operation,
and ongoing maintenance of the EMR are aligned
with AltaMed’s patient care quality and data gathering
goals. Requirements: Current
valid License as a Registered Nurse. Licensed
through the California Board of Registered Nursing.
Three plus years Clinical IT experience. Two plus
years Hands-on patient care experience in hospital
or clinic setting. Location: Commerce
CONTROLLER: The position
supervises all aspects of the Company’s
finance and accounting. Oversees budgeting and
financial forecasts, maintains internal control
procedures and analyzes and reports variances.
Prepares and/or oversees cost reports for regulatory
entities and prepares analysis
for year-end audit, in accordance with generally
accepted accounting principles. Requirements:

NURSE,
CASE MANAGER
The
Nurse Case Manager will provide monthly case management
services to an ongoing case load of individuals
living with AIDS. Works as part of an interdisciplinary
team coordination social work and mental health
counseling, psycho-social support services, in-home
support, legal services and skilled nursing to
men and women with AIDS who are homebound. When
applicable, the RN Nurse Case Manager coordinates
activities with the hospital nursing personnel,
with members of the interdisciplinary team and
with the physician in the clinic. The Nurse Case
Manager will report to the Director of HIV Services
or to the Case Management Supervisor, NCM works
in close collaboration with the Divisional Lead
Physician. Oral communication skills are necessary
to give clear, concise instructions to patients
and/or families regarding treatment, to consult
with medical team, to teach and represent recommendations
on programs, and to interact with department staff,
other departments, patients and families with
good public relation skills, and to understand
oral instructions. Requirements: Graduation from an accredited
Baccalaureate School of Nursing or a related filed

management or an RN with three years experience
in home health care and/or case management. 1

services to all of our customers. Should have
the ability to assess Senior Services and other
patient/referral needs (such as: Family Medicine,
Women’s Health, Pediatrics, HIV/AIDS, etc.). Will
work to identify new referral sources and will
